Always start off with 20-30 minutes of cardio and work your way up. Does your cardio have to be fasted?  No.  In a contest prep situation Hany recommends starting fasted cardio a few weeks into your prep at 12-16 weeks out and then increase cardio 30-45 minutes and you can add a second session.

You can go up to 60 minutes of cardio if you are really out of shape up to twice a day, but the key is not to get that far out of shape so that you are forced to do that.  You want to stay in the range of 20-30 minutes of cardio.
